Normalize the News tracks revisions in other outlets’ reporting. In the same spirit, NTN corrects its own errors in public.

How this ledger works: this is an append-only record of NTN’s own mistakes. Entries are never edited or removed once published; if a correction itself turns out to need revising, a new dated entry is added rather than changing the original. Every entry states what was wrong, the fix that was applied, and the two named reviewers who signed off before it was published. One entry below covers a false-positive revision that was published to the Revision Tracker in error and then removed.

What was wrong: A capture-noise difference (a rotating ad token) was briefly recorded as an article change before the confirmation gate filtered it out.
Fix: The noise-filter ruleset was updated and the entry was withdrawn. No article text had actually changed; the underlying article was unaffected.

Methodology page — substantive-change threshold wording
What was wrong: The published "substantive vs. minor" threshold was described imprecisely on the methodology page.
Fix: The methodology page was updated to state the exact word-count threshold used to classify a change as substantive.
